What are reasonable expectations and suggested alternatives?

Currently I have Internet service through my local cable provider and I am very satisfied with all aspects excepting upload/download speeds, which have fallen precipitously this year. Speedtest.net gave me a measurement of .39 Mbps download and .43 Mbps upload a few minutes ago.
