Reeve : Sounds like you are looking for an issue tracking system. MKS has Integrity Manager, which is well integrated into our Implementer product. Integrity Manager (written in Java, currently runs off the iSeries) allows you to create issues, assign them to developers, control workflow of the issues, when used with Implementer or Source Integrity (our NT/Unix source control product) it also keeps track of what code has been changed, and the workflow can control what the programmer can do to move the code around (i.e., if an issue hasn't be approved to be moved into production, the workflow would stop it from being promoted into the production environment).
