Any chance some PTF's were applied, or an IPL may have been performed
which may have updated PTF's?

Check the subsystem job itself to see when that started.
Or you can query the PTF Services.

I don't think there's anything in CHKPRDOPT to fix anything, but I could
be wrong. Anything which still lists 5716WP1 in the help as a sample to
check has to be a bit dated.

I have some instructions written up about CHKPRDOPT during our quarterly
downtimes.
Traditional failures have been a failure of just a few things:
- Some specific listed PTF's had to be applied
- I was missing a *SQLPKG I saved/restored from another system.
