I agree with all the logging and tracing suggestions, but there may be other
things going on if I understand your post below.

Do you mean the same physical attachment (like LPAR B is pulling a file from
LPAR A, or just similar content & size (assuming this...)?
Have all of these been accounted for (long list):
Did LPAR B ever send attachments via any email method, to any svc provider
other than Google?
Is LPAR B's path to the internet the same as LPAR A? Same firewall? Same
rules?
Is the file object that is the attachment have exactly the same
Ownership,Group authorities, Object authorities, data authorities, Path?
Is your profile on each LPAR same(you ran the job)? Same library lists, same
job descriptions , etc
Is this running under adopted authority? And all that needs to be checked.
Is there an exit program securing smtp on either system and have you checked
the rules.
Are your checking the email for virus on either lpar?
Is it a *signed doc or *encrypt?
Have you compared every single option on CHGSMTPA cmd?
Do you have the same OS and ptfs on each lpar? Up to date?
Have you been through this doc?

I'm sending to myself. If I send myself a message, it arrives. If I attach
anything substantial, no show.
I just tried with one single attachment, 1206 bytes. 170K, no good.
Works fine on LPAR A. Same box, LPAR B, messages never show up at
destination if there's the same attachment.

CHGSMTPA for a maximum attachment size.
Doesn't seem to be any setting for max attachment size.
