Silly question but have you by any chance accidentally moved this disk at any time without BIN mode? I had this exact problem and that was the cause.

It sounds like you have everything correct so I would initially focus on assuring that the media is good.

VIOS CLI command "lsmap -all" clearly shows the file mounted to vhost2 (adapter #46) and the backing device is /var/vio/VMLibrary/V6LICj.iso

All that said, whenever I attempt a D-mode IPL it immediately bombs because the load source is not found. Ideas anyone? Thanks
