I thought I covered that one. Oh well.

Yes, some omits for stuff like QADB* should be in there.
Yes, some omits for deleted rows < x would help performance.
Not sure how deleted records helps concurrent inserts.
Perhaps also some application uses deleted records to 'preallocate' space.
