Be sure to look at RJS iForm as an option. They have combined some open source tools with some custom tools. And very good support.

We have switched from Bottom Line ( product is Create!Form ) to Cybra ( product is Forms Magic).

I would emphatically steer you away from Bottom Line ... the support sucks.

We are replacing our forms vendor and I was wondering if anyone has any reccomendations and/or horror stories.
