I would suggest looking at our iForms product.

You can create forms from iSeries spool files, but also from database driven data sources, so spool files are not required.

Feel free to contact me offline or request a demo from our web site and our sales team can contact you to discuss your needs.

Also take a look at our WebForms product for doing web form based data entry. It's a nice way to eliminate paper based forms altogether.
