Well, the query debug messages show nothing that strikes me as remarkable. The CPI432C/D listed a bunch of what I'm guessing are SQL indexes (because the names end in the form, "I6," "I11," and so forth), most of them with reason 17, one with reason 4, and one ("...I15"), the one also mentioned in the CPI4328, with reason 0.

There don't appear to be any messages associated with the record that "slipped through."

They tried my suggestion of writing a simple RPG program accessing the file; their reply was a bit cryptic, but my understanding is that (1) RLA through ILE RPG worked, and (2) they couldn't do anything with OPM RPG, because the field names in the file were too long.

I'm going to ask them to describe what the "I15" object is, and also ask them if they can either send us a dummy library that can duplicate the problem, or give us temporary access to their box, or something similar (even if it's just a gotomeeting session), in hopes that maybe we might be able to get to the bottom of this.
