I have had this happen for a very long time (very occasionally), and am starting to get a few more. We are thinking it has do with users getting dual screens, and are switching apps more frequently. Some of the users said that they had been away from their session for several minutes, and when they switched back, the key functions were gone.
I just had a service call with IBM last week on this. We wound up re-installing Client access on those users machines (COMPLETE UN-INSTALL, REINSTALL).

Of course, since this happens very infrequently, it may take a few weeks to see if that takes care of the problem.
