First, do a WRKF QAOKP*. Do any of the physicals (hence the P) have a
number at the end like QAOKP05A01 or have the words "Old name" in their
description? Repeat for QAOKL*.

When you do a DSPFD qaokl03a do you see something like:

Was this one of those upgrades where you get in new hardware running
V5R4M5, take a machine running something earlier, save and restore it on
the new hardware and then try to get it back to V5R4M5?

If you do a GO LICPGM, 10. Display installed licensed programs, do you
see:
Licensed Installed
Program Status Description
5722SS1 *COMPATIBLE Library QUSRSYS
and the installed release (F11) is equal to V5R4M0? (only 5722999 goes to
545 and that's not on this screen)

I did some searching and didn't find an applicable ptf otherwise for 540.
But since you are on a currently support version you might be able to call
800-ibm-serv and see if they can help you out. Have your DSPSYSVAL
QSRLNBR handy.
