Unfortunately, not much can be done about that.

I believe that, if the user chooses to receive digests in MIME mode (so each message is included in the digest as an attachment) some mail programs will let them reply correctly. But if the digest is in plain text, there's nothing that can be done.

That said, if the user sets the subject correctly, the list archiving software will do it's darnedest to guess what thread it belongs to.

I wonder then, why do Birgitta's replies never seem to be threaded properly in the NG? Is the software being used lacking an AW: as synonym of RE:? Or do these comments only apply to Archive threading; i.e. not about the NG threading? If the comment about the "Mail programs" suggests the problem is the client, how would a client providing the /reply prefix/ not be aware of what it is doing?

The news server interface (news.midrange.com) is somewhat weak in that area ... I believe the next major version of Mailman has that targeted to be beefed up.
