I hear you. Good points.

In theory these practices make you more productive (I haven't seen any evidence of this, yet), improve quality, and save money. However, an additional first effort needs to be made in order to get something like this started, and, even if there is a mid- or long-term benefit, usually there are no free resources now to start something like this.

On top of that, you need to sell this novel ideas to users, customers, and management, and that can be hard (or impossible).

On the other hand, let's face it... we have varying degrees of laziness and resistance to anything that's new and may have some risk. And that works against us.

but, like many other companies, we could not support this method:

If your company has been thru any of the "down sizing", "resizing",
"right sizing" philosophies and current personnel are now doing
the work of 2-3 people, time is a luxury they can't afford :(
Unless, of course, the new application is going to eliminate a
warm body...

I was being a little facetious with regard to my "windows" remark <grin>
I will admit, that I have seen *some* windows-based projects go very
well as far as application development and deployment is concerned.
But they seem to be the exception rather than the rule ;-)

plain poor implementation... maybe lack of knowledge,
experience, money, or time. In any case, it's bad, but it
happens everywhere, regardless of the platform.

Good QA practices are not old dinosaur stuff... indeed, have
you looked in the recent practices about test-driven
development and agile development and testing? I am
impressed. I confess that I don't use them (yet) in my shop,
but it actually goes beyond the typical approach (making
somebody test your stuff by playing with it in a safe testing
environment). Here are a couple links to modern testing
