It is probable that a clear explanation would be required, for understanding what transpired when the "client did a V5R4 upgrade and ran into some problems and ended up doing a restore before the upgrade." The explanation of that scenario may expose the condition of missing data, as not being limited to the service table entries. However I offer:

Given there is only the one file, the results of the first step still have value; i.e. the request to: dspfd qusrsys/qatocps *mbrlist Is there just one member named SERVICES with data? If there is data in the one member, then having no data show in WRKSRVTBLE would suggest an error in the code for its open+read, or the issue is limited to specific entries; a trace would be the approach. Or...
Along with that data I would also review:
dspobjd qusrsys/qatocps *file *service output(*print)
dspobjd qusrsys/qatocps *file *full output(*print)
dspfd qusrsys/qatocps *all output(*print)
From that information, details like create, change, save, & restore dates, owner & creator can be reviewed for how the file might have come to be [empty] on the system. An effective match for restore and change dates for the member would indicate likely the file was restored as part of install of QUSRSYS; in any case, review of that joblog [GO LICPGM option 50 gives install history; F1=Help, F9=Details to get job name] might have value -- but restored empty would suggest the file was missing before the install. If instead the restore date is from the 'restore before upgrade', then the file was likely either restored from a backup taken before the missing service table entries were added, or a restore after a scratch install did not include the file; e.g. due to differences when ALWOBJDIF(*NONE) was specified.
