I downloaded formMail.pl (the perl script) just to read it and see what it does. You might want to inform the list what error(s) you're getting. I noticed the following line:

$mailprog = '/usr/lib/sendmail -i -t';

Is file "/usr/lib/sendmail" found on the IFS?
