Number of reasons, some due to OS issues, some due to abnormal IPL, other
reasons too (i.e. sometimes due to premature kill of index rebuild).
Get current on PTFs to help prevent it from happening again and run RCLSTG

Folks:
After espousing the virtues, stability, and reliability of DB2/400 for
years to the developers & QA people at our other office ... we
encountered a problem with the database that I can't explain.
I found a CPF32A6 - Cross-reference notification 1131 for request 90,
problem type 1 error on the QSYSOPR message queue.
The database collection the error referenced has been on the system for
a while ... without having any problems.
The only thing that is unique about this database collection is that
it's only accessed using SQL via JDBC.
As far as I know, we haven't had any system failures or power outages.
Any thoughts on what might have caused it?
