Hi Mike

We use @riadne Software's CoolSpools -
http://www.ariadnesoftware.co.uk/CoolSpls.htm - which can append PDFs
as they are converted from spooled files, or just merge PDFs created
by CoolSpools or any other product. It has good support for bookmarks,
particularly if you use the DDS DOCIDXTAG keyword to set the values
you want.
