We are using MessageLabs, contracted through IBM Security Services and have been very happy with it. You basically have your external DNS MX record pointed to MessageLabs and they filter the mail and then pass it to you (quickly with virtually no delay). You can also filter your outbound mail by using them as a relay host. They also offer antivirus mail scanning that is better than most in house solutions because it does better at catching the 'zero hour' infections (an in house signature file based system may not pickup the new virus signature until the next day). One previous recommendation was using a third party managed email filter company, but I can't find archives references to who was recommended. This client is currently running the OS/400 smtp server, with Byteware Standguard for anti-virus. I don't think they'd be too open to a Linux box as an email filter, because they don't have anyone on staff familiar with it. What are other people recommending?
