If you want PDF output and don't mind passing through a PC, check out pdf995 (http://www.pdf995.com/) or perhaps OpenOffice's PDF support. For native PDF creation we use JetForm's/PDF Wizard. It's OK in that the created PDFs are nice but the product is a bit of a CPU hog. Adobe bought JetForms a while ago; that doesn't seem to have done a lot for the product. :)
