(almost definitely sure of that...) 2 - Hipers are High Impact and Persvasive PTFs... they are of 'serious' import and have been found to have a widespread impact if not applied. Having answered #2, it's been my practice to hold off and load Hipers and such when doing any widespread PTF work (installing a new LICPGM or complex app, or definitely with a cume). Even with the pervasive nature of them, it's been rare for me to actually run into the 'need' for them (your mileage may vary...). I consider Hipers to be very much the same as a cume package, you should stay up to date with them as the opportunity arises to get them applied. As a rule of thumb I think they are probably always applied as the opportunity permits.
