I always assumed they were physical, to let the post office do the first
sort for only the cost of a PO Box.

True, but look at a zipcode with a rebate fulfillment house in it.  Each
rebate campaign uses a different PO Box.  I imagine they're logical and
not physical, although I couldn't say for sure.
 
Anyway, I've seen 5 digit PO boxes many times.  In those cases the zip+4
is not unique as the last 4 digits aren't sufficient to ID the PO box in
question: xxxxx-0102 works for box 102, 10102, 20102, etc.
