Al,

You never heard of that radiation machine that would overdose patients with
something like 100x the normal dose because of a software bug
(http://courses.cs.vt.edu/~cs3604/lib/Therac_25/Therac_2.html)?  Ever heard
of a pacemaker recall
(http://www.researchmatters.harvard.edu/story.php?article_id=286)?  Mistakes
like this do happen with dialysis machines, pacemakers, etc.
