I don't know whether anyone else has mentioned this, but one situation in which I've encountered this form of horror is when you have an attention key handler program that can terminate in such a way that it sends an *ESCAPE message - e.g. if the attention key handler puts up a series of panels & doesn't handle device errors properly. This seems to upset the interactive job badly.
