In both cases it was because of a misconfigured backup mail exchanger (MX). Please, if you feel you need to use a backup MX, make sure it's properly configured. Also, keep in mind that most mail systems will try to send mail for a number of days before giving up and returning the message ... unless you expect your mail server to be off line for an extended period of time, there really isn't a pressing need for a backup MX.
