I have run this scenario (and made it work) in a few ways:

1. Twinax balun attached to a port on twinax brick, UTP to terminal with
twinax balun on T-connector.

2. Twinax balun attached directly to system (i.e. on a 4xx system), UTP to
terminal with twinax balun on T-connector.

3. Twinax cable to Twinax > UTP active or passive hub/panel, then UTP to
the twinax terminal with a balun on the T-connector.

I would recommend you only use the direct connect strategy (i.e. NO twx to
UTP hub or panel) only for short distance runs, say within the same room.
If you are going to have any distance involved at all, or if the cable is
likely to get any RFI (such in a warehouse) then I would highly suggest an
ACTIVE UTP hub. The signal will possibly need to be boosted, especially in
this type of environment. The passive panels out there don't necessarily
prevent signal loss, which in turn causes the terminals to have erratic
behavior.
