Many years ago I had bad diskettes in an upgrade to the old S/36.
The explanation was that IBM had farmed out diskette production to some 3rd 
party that lacked IBM quality control.
IBM got so many complaints that they brought the work back in-house, but I 
could see them wanting to back trace bad media to whatever produced bad media.
