My e-mail server gets around the problem by requiring remote connections 
to get mail first.  When we get mail we open the relay for 15 minutes. 
During the 15 minutes we can send mail, but then the relay closes again.

Jon,
If you are accessing the mail server via the internet then you are using a
relay and the spammers can utilize it. You will either need to dial in to 
a
remote access server of some sort or use VPN to provide you with a valid
internal IP address that can be set to allow relay.
