On our 170 box when we went from V4R3 to V4R4 and when we went to V4R5
after loading the first upgrade CD and processing the console came up
with an error.  I am not sure of the exact error but it basically said
that it was unable to install from the CD.  After restarting a couple of
times I took out the first CD and placed the next one in.  Upgrade
completed successfully from there on.  I know I didn't answer any
prompts but it tried to process the first CD twice.

Maybe something similar is happening in your situation.
