I've been burned on it too.  I used Ops Navigator and the case and 
extensions look the same in the source code and in the directory.  I'll 
look again though because this behavior is exactly what you are 
describing.

i've been burned on the case part - more than once.  sometimes photoshop
will change the extention to caps and the original html document had it
in lower case.  they will show on the web tool but not in a browser.
