For example a former employee of M$ in an IBM class, a consultant talking to other people about stuff that should have been kept confidential. Check the archives - this topic has come up several times before, with different people supplying snippets of info. What stuck in my memory was the notion that when you are dealing with Terrabytes of information scattered on many different servers it takes a while for business rules to be locked into place & no one actually did the math to figure out how long it should take, they just gave up after waiting for several days & M$ management was told they could have the data but no enforcement of their business rules & that was unacceptable. They had no idea if it was going to work if they just waited another week. They did no small scale testing to find out what the problems were. They made no attempt to solve the problem for the customers of M$.
