Hi, I use to work for am IBM business Partner in Central America who replaces completely IBM (They left CA in 1992) as an AS/400 SE. As I recall, there was a batch of 6606's that had serious problems. IBM sent us a batch a new 6606's to replace the ones that they had installed in Guatemalan machines. We had to change 25 of them. I don't have the information anymore but it was a batch of disk produced on Rochester in 2 months period. Maybe you can contact your IBM representative and ask for a list of serial numbers of this disk's and replace them ASAP.
