QtmhCvtDb difficulty

I'm trying to create a simple CGI in RPGLE that retrieves form data, makes
decisions, and does a little something.  After QtmhRdStin, RcvDta is
populated with unparsed data like truck=blah&message=blah+blah.  It is the
QtmhCvtDb phase that seems to be failing.  No errors are generated - it just
doesn't fill the externally described data structure with useful values.
I'm V4R3.  I'm beginning to wonder if I need a PTF.  If someone has a simple
retrieve-data-from-a-form RPG example, I would love to see it.
