Check your termination on the twinax line. How many devices on the controller? Do you have duplicate addresses in use? I would start by disconnecting all the terminals in the chain and testing the twinax back to the controller with a port tester. Then add one cable at a time to the chain testing with the port tester. 1.) most likely bad termination. 2.) most likely to devices with same address 3.) most likely terminal emulation and device description not matching.
