This sounds like the problem I have with remote outq spool files going to hold. I called Support Line and they said that this is a known problem on all RISC systems (v3.7 up) and an open APAR exists but no PTF yet. It seems that some applications write a temp splf immed prior to creating the real one (perhaps AFP, or forms with overlays, etc.). The RISC systems are so fast that the 'see' the temp file but don't quite catch it and the real file ends up going to hold status thinking its send failed. The work-a-round is to end the writer release the splf and strrmtwtr again.
