I have a document called "Document Content Architecture: 
Final-Form-Text Reference SC23-0757".  The version that I have was
revised in May, 1985.  I got it about two years ago from IBM although it
was a tremendous hassle.  I called the IBM Publications 800 number and
they relayed a message to what they call the "controlling party".  He
called me back from Germany and said he'd mail the document to me. 
There was no charge although the whole process took about a month!

It's printed on A4 paper and has a blue cover -- It doesn't look like
one of their regular publications.

You can try going this route.  You have to be *very* persistent.  Or,
you can email me with a specific question and I can look it up and tell
you.
