Machine Index Concurrency

Does the OS handle concurrency issues related to machine indexes or does a lock have to be issued to ensure integrity of the object during inserts, updates and deletes? Since space locks are not enforced implicitly by the machine what about concurrent users on the same index, any issues other than a dirty record situation?
