I have experienced it several times on release 7 but have never been
able to pinpoint the problem.  The only thing I could determine was the
item was a kit and the components caused the problem.  I haven't been
able to recreate it in over a month so I don't know if a PCM or PTF
fixed the problem.  Hope this helps.
