We are also on 6.0.04, mixed mode. We have had this problem on very rare occasions (4 out of 200,000 ILN records). When I called SSA, they told me that the problem may be related to O and/or RD transactions. They also stated that there was a BMR to fix the problem. In our case, the O or RD could not have caused the problem. Since the problem was so rare, we chose to simply delete the 'wrong' ILN and ILI records and periodically monitor the files for duplicates (we haven't pursued the BMR). I hope this helps.
