This topic comes up regularly.  Look back through the Archives at midrange.com.  I think there were a couple of responses that provided something close to what you are looking for.  If I remember correctly George Sagen provided an Access database back in April 1999.

SSA Canada has a series of Excel Spreadsheets which outline DB Relations,
Files, Programs and Fields. They make them available, on request, to their
Canadian customers. I also know from talking to them that their Chicago
office has something similar. These are for Version 6.1.01 but most
information should apply to earlier versions. Hope I'm not stirring up a can
of worms. I'm new to BPCS.
