Juan Rodriguez of Nexgen, Inc.  has put together a very good document on "
Using the DBMON Monitoring Tool to Improve SQL Performance "

indices recommendations at the end of the document.  I have not tried them
myself yet. We also created the following additional logical files that help
the billing process:
