I found a |HostnameLookups On directive. It didn't seem to make a difference but it was a good guess!

This sounds suspiciously like an issue with an Apache configuration directive. I seem to recall something about a directive that could be used to bypass DNS checking, or something like that. I don't recall the directive name off hand. But the symptoms sound familiar.
