Could a non-reporting communication IOA cause this SRC Code to appear???

According to the operator who reported the problem, the IBM Tech told
him that it was an IOA that did not report on time.  I've been trying to
determine what that means.  

There is no entry in the Problem log for that time.  Still trying to
figure this one out.

Also WRKPRB should have it... It's most likely a Disk or Disk IOA
problem. 
I've also seen this code when the System Password has expired, but that 
shouldn't be the case here...
