We are using  a product from  "ACOM Single Source" call Quickcheck 400 and
QuickForms. The Quickforms software is a windows based product which allows
you to design your forms from scratch or scan  in forms. The forms can then
be uploaded to the AS400 where you can use the Quickcheck 400 software to
map an existing AS400 spoolfile into the form. The product will work with
any standard PCL5 compatible printer. We are using it with HPs using TCP/IP
passthrough printing.   I have found the product to be fairly easy to use
and stable. The cost was just  under $10,000 U.S .   Another product we
looked was  CreatePrint which seemed pretty good as well, although it used
PostScript Printing language.
