Speaking for Germany you have different choices:

1. Use one of the online-services AOL or CompuServe. They have nodes in 
all major cities.
2. You might use T-Online which is run by Deutsche Telekom. 
You have access via the same local access number nationwide.
3. There are some nationwide ISPs. For example Eunet ist owned by an US 
company (UUNet) and offers services in whole Europe incl. East Europe 
and Russia (http://www.eunet.de, in German and English).
