Printing Headers in V5R1 -- CODE400-L

I've just installed V5R1 and when I print from the editor it no longer puts a heading at the top of the page. I want the file name and a page number, like I was getting in the V4 version.

I can't find anything in the help that tells me how to put this back. Can someone remind me how?
