I suspect that the include approach would perform marginally better but probably not by a lot.

Personally I've done a mixture of both. If there are large blocks of straight HTML I inline it - if there are lots of conditional "bits" I tend to use print/echo because it is less messy.

What I should do of course is use Smarty (or similar) for all of it but most of my stuff is for short term use so maintenance is not an issue.
