The other issue I've had a lot over the years is hard errors in my RPG and
thus the headers (nor anything else) gets sent back to the browser. In
short, check the CGI joblog of the Apache instance you are running against
to see if there are hard exceptions that aren't being caught. I always try
to make sure and put a MONITOR block around the mainline of each CGI
program.
