Besides the +Includes option, I have the following:
AddOutputFilter INCLUDES .shtml AddOutputFilter INCLUDES .htm AddOutputFilter INCLUDES .html AddOutputFilter INCLUDES .pgm
My HTML pages have things like <!--#include virtual="/navigation1.htm" --> in them. I see that you have "include virtual" in uppercase. Have you tried making it lower case? Also, if you're on V5R1 or V5R2, make sure that you've installed the group PTF's for 5722DG1. There were problems with SSI's in the initial release of the server.
