I got it working. It turns out I had my config file set up wrong. I was
mapping to QSYS.LIB/QHTTPSVR.LIB/DB2WWW.PGM, but I have our production
version and it's customized INI file in QSYS.LIB/CGIBIN.LIB/DB2WWW.PGM.
Once I switched my references in Apache to point to my customized INI
file it served the pages just fine. Thanks for all the help, your
questions and comments helped me to figure out what I had done wrong.
Sometimes the obvious mistake is impossible to see.

I choose the first one. That's the correct syntax.

So your macro vendor.mac resides in /WWW/APACHEDFT/CGI-BIN?

Your INI file with its member DB2WWW is in library CGIBIN?
