Not quite correct. I also have licensing via ESS. I went to the license
key center at:
https://licensing.subscribenet.com/control/ibmr/login
Where I logged in and could download my license key activation kit for
9.1. The only issue I have is that Installation Manager took a very long
time to allow me to find information before the license was imported. It
did get there and now I am licensed for 9.1.

You're not prompted for the other disks. It normally finds them if
they're in the same root directory.

I managed to get disk2 downloaded, did the install but never got prompted
for anything on disk2.
For the benefit of others, what does disk2 contain and what is it used
for?
One other note to pass along - I was able to go into RDi 9.0.1 and look
at the Available Sites for Installed Programs and was able to export them
into bookmark.xml files that were easily imported into RDi 9.1. This let me
easily add things like SQL Exlporer, iSphere, Softlanding plugins etc into
RDi V9.1
